A small percentage of patients in methadone (MTD) maintenance treatment programs show a very fast metabolism of methadone to first metabolite EDDP, resulting in low methadone concentrations in the urine (“fast metabolizer”). As a result of the low methadone concentration, those patients appear to be methadone negative. Testing for EDDP in these cases leads to positive results, confirming the compliance of the patients.
