Ordering Product If you would like to place an order, please download, complete, and print the DrugCheck / Express Diagnostics Order Form and fax it to 888-444-5754. If using a purchase order (P.O.), you may email your order to orders@drugcheck.com (for security reasons, credit card information should not be emailed). Ensure that P.O. orders include the information requested on the DrugCheck/EDI Order Form.
